Regional Liaison (RN/LPN Preferred)
Location: Nashville, TN
Champion Care is seeking a motivated Regional Liaison to join our growing team. This is a great opportunity for a nurse who enjoys blending clinical judgment with relationship-building to help patients transition safely to the right level of care. In this role, you’ll be the key connection between hospital teams and our skilled nursing facilities, supporting patients and families while strengthening referral partnerships.
What You’ll Do
Build and maintain strong referral relationships with discharge planners, case managers, physicians, and hospital staffAssess patients and review medical records to determine clinical appropriateness for skilled nursing admissionEducate patients, families, and providers on Champion Care services and the benefits of skilled nursing and rehabCoordinate the admissions process to support timely, safe transitions from hospital to facilityPartner with admissions, clinical, and marketing teams to meet census and growth goalsRepresent Champion Care through hospital in-services, presentations, and networking opportunitiesWhat We’re Looking For
Experience in a hospital liaison role, admissions, care transitions, or healthcare sales/referralsStrong understanding of Medicare, Medicaid, and managed care guidelinesExcellent communication, organization, and follow-throughAbility to travelWhy Join Champion Care?
