We are seeking a qualified Physical Therapist Assistant to treat patients of all diagnosis to relieve pain, develop or restore function and maintain maximum performance, using physical means such as exercises and modalities as indicated. The goal of home health care is to help our patients obtain a faster recovery, regain independence, and become as self‑sufficient as possible to help prevent rehospitalizations.

• Provide PT services to patients in their homes, including exercise prescription and modality application.
• Assist patients in achieving functional goals and improving independence.
• Collaborate with overseeing Physical Therapists and interdisciplinary team to ensure care quality.
• Document patient progress and maintain accurate records.

• Current, valid, and unencumbered Tennessee Physical Therapist Assistant license.
• Current CPR certification, or within 30 days of hire.
• Valid driver license, reliable transportation, and auto insurance.
• One year or more clinical experience preferred.
• Self‑motivated and customer‑service oriented.
• Must be able to pass background and drug screening.
