Palliative Care Advanced Practice Provider - Hospital Based
Location: Tri Star Centennial Medical Center, Nashville TN
Status: Full-Time | Monday–Friday | 40 hours/week
Reports to: Vice President, Clinical Operations & Palliative Care
About This RoleJoin Alive, Middle Tennessee's original hospice and a trusted leader in end-of-life care for 50 years, as a Palliative Care APP embedded at Tri Star Centennial Medical Center. You'll serve as an inpatient palliative care consultant, helping patients and families navigate complex illness through expert symptom management, goals-of-care conversations, and advance care planning. This is an opportunity to build meaningful relationships with a single hospital team, grow your own referral base, and practice at the top of your license with the backing of a respected, mission-driven organization.
WhatYou’ll Do
- Provide palliative care consultations alongside hospital medical teams, managing symptoms related to acute and chronic conditions
- Lead compassionate conversations with patients and families about goals of care, prognosis, and treatment options
- Develop and implement individualized care plans in collaboration with physicians, social workers, chaplains, and case managers
- Build and nurture referral relationships within your assigned hospital
- Order and interpret labs, imaging, and medications; document thoroughly in the medical record
- Serve as a resource and mentor to students, residents, and new team members

- Active, unrestricted Tennessee license and national certification
- Unrestricted DEA
- Valid driver's license and insurance
- Experience in hospice, palliative care, ICU, or inpatient acute care
- ACHPN or HPCC certification (or willingness to pursue)
