The Case Manager assists with coordination of services through the continuum of care and in balancing quality patient care with cost‑effective utilization of resources.

• Coordinate admission, treatment, and discharge plans and ensure proper utilization of hospital services
• Ensure care is provided at the most appropriate level to meet patient care needs
• Serve as a liaison to ensure appropriate reimbursement for hospital stays and services

• Demonstrate mastery of nursing patient assessment and evaluation in complex clinical settings
• Provide patient education independently and without requiring additional support
• Integrate evidence from multiple sources to determine practice changes
• Demonstrate mastery of case management and treatment planning in complex clinical care coordination
• Deliver treatment planning services without requiring support or instruction
