Licensed Practical Nurse | PRN | Nashville, TN
All Heart Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our home health team in the Nashville area. This PRN position offers flexible, as-needed scheduling while providing personalized, high-quality care to patients in their homes.
Key Responsibilities:- Provide skilled nursing care to patients in accordance with the plan of care and physician orders
- Monitor patient health status and report changes to the supervising Registered Nurse or physician
- Administer medications and treatments as prescribed
- Assist with wound care, vital signs monitoring, and other clinical tasks
- Educate patients and caregivers on disease management, medications, and home care techniques
- Maintain accurate and timely documentation in compliance with company policies and regulatory standards
- Collaborate with the interdisciplinary care team to ensure coordinated patient care
- Travel to patient homes within the assigned service area
- Current Licensed Practical Nurse (LPN) license in the state of Tennessee
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong clinical and communication skills
- Ability to work independently and manage time effectively
- Previous home health experience preferred but not required
- PRN (as needed)
- Flexible scheduling based on patient needs and coverage requirements
