All Heart Home Health is seeking a compassionate and dependable Full‑Time Registered Nurse (RN) - Field Nurse to provide high-quality, patient-centered care in the Nashville, TN area. This role is ideal for a nurse who enjoys working independently while making a meaningful impact in patients' homes.

• Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
• Perform comprehensive patient assessments and ongoing evaluations
• Administer medications, treatments, injections, and wound care as ordered
• Educate patients and families on diagnoses, medications, and care plans
• Accurately document all patient care and progress in the electronic medical record
• Collaborate with physicians, case managers, and interdisciplinary team members
• Monitor patient condition and report changes promptly
• Ensure compliance with state, federal, and All Heart Home Health policies and procedures
• Participate in on-call rotation as assigned (if applicable)

• Active Registered Nurse (RN) license in the state of Tennessee
• Graduate of an accredited nursing program
• Minimum 1 year of nursing experience (home health experience preferred)
• Strong clinical assessment and critical‑thinking skills
• Excellent communication and time‑management abilities
• Ability to work independently in a field‑based role
• Valid driver's license and reliable transportation
• Current CPR/BLS certification
