Position SummaryThe Wound Care Nurse is responsible for the assessment, treatment, and ongoing management of residents with wounds, pressure injuries, and skin conditions. This role works closely with the interdisciplinary team to ensure optimal healing outcomes and compliance with regulatory standards.
Key ResponsibilitiesPerform comprehensive wound assessments and develop individualized care plans
Provide and oversee wound treatments, including dressing changes and infection prevention
Monitor healing progress and adjust care plans as needed
Collaborate with physicians, nursing staff, and therapy teams
Ensure accurate and timely documentation of all wound care services
Educate staff on proper wound care techniques and prevention strategies
Maintain compliance with all state, federal, and facility guidelines
Active LPN or RN license in good standing
Previous wound care experience preferred (SNF or long-term care a plus)
Wound Care Certification (WCC) preferred or willingness to obtain
Strong clinical assessment and critical thinking skills
Excellent communication and teamwork abilities
