Advanced Practice Fellow (NP/PA) – Working within a multidisciplinary team across inpatient and outpatient settings to deliver evidence‑based, patient‑centered care. The role includes clinical assessment, diagnostic planning, therapeutic interventions, and professional development within the Leadership and Education in Neurodevelopmental Disabilities (LEND) and Leadership Excellence in Advanced Practice (LEAP) programs.
Job SummaryThe Fellow will perform complete history and physical examinations, order and interpret diagnostic studies, manage treatment plans, and document care in compliance with regulations. Participation in daily huddles/rounds, collaborative decision‑making, and mentorship of students and nursing staff are integral components of the position.
Key Responsibilities- Assessment of Health Status
- Complete history and physical examinations.
- Order and collect data using appropriate assessment techniques.
- Identify risks such as medication side effects, immobility, and nutrition deficits.
- Participate in daily interdisciplinary huddles or rounds.
- Diagnosis
- Collaborate with the team to establish accurate diagnoses of acute and complex chronic conditions.
- Order, interpret, and supervise diagnostic tests.
- Formulate differential diagnoses and identify appropriate interventions.
- Plan of Care
- Develop individualized, evidence‑based care plans for acute and chronic needs.
- Prescribe diagnostic strategies and therapeutic interventions, including pharmacologic and non‑pharmacologic measures.
- Perform advanced procedures within privilege scope.
- Communication & Collaboration
- Maintain continual communication with the interdisciplinary team.
- Facilitate patient, family, and staff communication across care settings.
- Document key progress in daily notes, transfers, and discharge summaries.
- Professional Practice
- Provide preceptorship and education to students and nursing staff.
- Engage in research and evidence‑based practice analysis.
- Attend Grand Rounds, council meetings, and continuing education sessions.
- Master's degree in Nursing or related field.
- Less than 1 year of relevant clinical experience.
- Clinical knowledge, sound judgment, and professional communication skills.
- Ability to work independently and within a team setting.
