Advanced Practitioners Needed

Ballad Health Medical Associates, a member of Ballad Health, is currently seeking experienced, motivated Advanced Practitioners to join our team of Primary Care providers in delivering home-bound patients with routine medical care — including visits related to transitions of care, annual wellness exams and providing structure for primary care.

What we're looking for:

• Ability to work independently
• Experience in care of older adult (geriatric) patients — preferred
• Home care experience — desired
• Excellent administrative, organizational, and verbal skills — preferred
• Effective communication skills, especially with older adults — preferred
• Previous EPIC experience desired but not required

Position details:

Our at-home health care program is a team effort between our physicians and advanced practice providers that is designed to provide care for and support of Ballad Health Medical Associate patients by providing an additional level of medical care within their homes. Our overall goal is to help reduce/eliminate unnecessary hospitalizations/readmissions and prevent future health complications that can occur due to transportation issues.

In this role, you will:

• Work with the Coordinator to see patients based on a schedule
• Provide patient assessments including:
• Past medical history
• Review of symptoms
• Physical examinations
• Medication reviews
• Health risk assessments
• Screening and addressing any other disease-specific concerns
