At Abe’s Garden, we are establishing a national, evidence-based model of residential and day/evening care programs for those living with Alzheimer’s disease and related dementias. POSITION SUMMARY: Provide personal care, assist in recreational activities, perform assessments, and give companionship to the residents of the Abe’s Garden.

• Direct and supervise the Care Partners working within Abe’s Garden, and provide training and guidance as needed.
• Observe, record, and report resident behaviors, symptoms, or changes in resident’s record and to oncoming shift.
• Perform physical assessments and obtain/maintain resident health histories.
• Record resident medical information and vital signs, note changes, and pursue more specific investigation as needed.
• Administer medications and other treatments to aid various conditions.
• Keep written documentation of services provided, as well as communication about high risk behaviors and/or critical incidents.
• Respond to and treat resident emergencies.
• Educate residents and their families about their conditions while establishing trust and rapport with them.
• Provide companionship and emotional support to residents and their families.
• On an as-needed basis, assist residents with daily activities, including but not limited to bathing, grooming, toileting, eating, dressing and engagement activities.
• Act as liaison between resident, family, physician and ancillary services.
• Maintain continuing education necessary for keeping required credentials.
• Maintain regular and timely attendance.
