Overview

This full-time, on-site Family Nurse Practitioner role is based in Hermitage, TN. The Family Nurse Practitioner will provide comprehensive primary care services, including health assessments, diagnosis, treatment, and management of acute and chronic conditions for patients across the lifespan. Daily responsibilities include conducting patient exams, ordering and interpreting diagnostic tests, prescribing medications, and developing individualized care plans in collaboration with physicians and other healthcare professionals.

The role also involves providing patient education, promoting preventive care, and coordinating referrals to specialty services as needed. The Family Nurse Practitioner is expected to contribute to quality improvement initiatives and support AdvancedHEALTH’s commitment to evidence-based, patient-centered care.

Responsibilities

Provide comprehensive primary care services including health assessments, diagnosis, treatment, and management of acute and chronic conditions for patients across the lifespan. Conduct patient exams, order and interpret diagnostic tests, prescribe medications, and develop individualized care plans in collaboration with physicians and other healthcare professionals. Provide patient education, promote preventive care, and coordinate referrals to specialty services as needed. Contribute to quality improvement initiatives and support evidence-based, patient-centered care.

Qualifications
  • Strong foundation in Primary Care and Medicine, with the ability to manage both acute and chronic conditions.
  • Familiarity with Medicare Advantage Plans [AWV, EEs, + PAFs].
  • Experience with Urgent Care settings and timely management of non-emergent urgent medical needs.
  • Current Nurse Practitioner license in the state of Tennessee and national board certification as a Family Nurse Practitioner (or eligibility).
  • Graduation from an accredited Nurse Practitioner program;
    Master’s or Doctoral degree in Nursing preferred.
  • Commitment to evidence-based practice, patient-centered care, and collaboration within a multi-specialty team.
  • Strong communication, clinical documentation, and interpersonal skills, with the ability to work effectively with diverse patient populations.
