** Works in collaboration with the RN or a licensed healthcare team member to provide direct care to patients, both pediatric and adult in the home setting..

* Department Summary:
** Vanderbilt Home Care Services – Intermittent Care provides short-term, medically necessary services to patients in their homes under physician direction. This division focuses on supporting individuals recovering from illness, surgery, or acute exacerbations of chronic conditions, allowing them to receive skilled care while maintaining independence in a familiar environment.
Intermittent services are delivered on a part-time or as-needed basis and include
** skilled nursing, physical therapy, occupational therapy, speech-language pathology, and home health aide support**. Care is tailored to each patient’s individualized plan, with an emphasis on restoring function, improving mobility, managing symptoms, and promoting self-care.
The program primarily serves patients transitioning from hospital or post-acute settings, making it a critical component of the
** care continuum**. Clinicians collaborate closely with physicians, case managers, and caregivers to ensure coordinated, seamless care. Frequent assessments and care plan adjustments are conducted to align with patient progress and evolving needs.
Vanderbilt’s Intermittent Home Care team prioritizes
** quality outcomes, patient safety, and reduced hospital readmissions
** through evidence-based practices and proactive care management. The use of advanced technology, including electronic health records and remote monitoring tools, supports timely communication and enhances care coordination.
By delivering high-quality care directly in the home, the Intermittent Care program helps patients regain independence, improve health outcomes, and transition safely to long-term wellness.

* Key Responsibilities:

* • Collaborates with patients/families and health care providers in the delivery of health care services
• Partners with the healthcare team by following established plan of care to ensure patient progress for effective outcomes
• Support plans to promote health and a safe and healthy environment
• The responsibilities listed are a general overview of the position and additional duties may be assigned
• Navigates intense medical scenarios with autonomy, under the direction of the supervising RN while maintaining compliance with the physician ordered plan of care
• Administer intravenous fluids and perform sterile procedures
• Recognize and respond to clinical situations appropriately and timely to prevent patient decompensation and injury
• Identify and assess patient needs and clinical changes, accurately reporting assessment findings in a timely manner to Supervising RN and physician
