Care Rite Centers is seeking a compassionate Wound Care RN/LPN to provide specialized wound management within our Healthcare & Medical Services department. In this role, you will assess, plan, implement, and evaluate advanced wound care treatments for residents in a skilled nursing and rehabilitation setting. You will collaborate closely with an interdisciplinary team to develop individualized care plans, educate residents and families, and ensure accurate documentation in accordance with clinical standards and regulatory guidelines.

Responsibilities
- Perform comprehensive wound assessments and develop individualized treatment plans
- Provide and monitor advanced wound care therapies and dressings
- Collaborate with physicians, therapists, and nursing staff on wound management
- Educate residents, families, and staff on wound prevention and care
- Document assessments, treatments, and outcomes in accordance with policy and regulations
- Participate in interdisciplinary care conferences and quality improvement initiatives
- Monitor and report signs of infection or complications promptly
- Ensure compliance with infection control and safety standards
- Support orientation and ongoing training related to wound care best practices
