• Provide skilled nursing care in accordance with each client's individualized care plan
• Monitor, document, and report vital signs and overall client condition
• Administer medications and treatments safely and accurately as ordered
• Observe for changes in condition and communicate findings to the registered nurse or care team
• Maintain clear, timely, and accurate clinical documentation
• Educate clients and families as appropriate
• Deliver compassionate, respectful, and client centered care in the home setting

• Current, valid, and unencumbered Tennessee Licensed Practical Nurse license
• Current cardiopulmonary resuscitation certification
• Strong communication, organization, and time management skills
• Ability to work independently while collaborating with a care team
• Reliable transportation for home visits
• Previous home care experience is preferred but not required
