Basic Function:
The Care Manager provides case management to older adults participating in Fifty Forward’s Living at Home, Care Team, and Conservatorship programs. This role includes in-person client visits, conducting assessments, coordinating resources, providing advocacy, submitting documentation, and supporting departmental operations.
Core Responsibilities:- Administer comprehensive functional assessments for clients accepted into case management programming to determine eligibility, needs, and appropriate services.
- Develop and implement individualized care plans, modifying them as needed, with attention to current and future needs.
- Assist in the completion of conservatorship interim and annual accountings.
- Provides after-hours on-call telephone coverage on a rotational basis.
- Connect with colleagues, community partners, and governmental agencies to ensure individualized, holistic support for each client.
- Submit referrals and applications to link clients to applicable resources.
- Respond to resource line inquiries and connect callers with appropriate services.
- Incorporate evidence-based practices into client care and decision-making.
- Maintain accurate client records and submit all required reports in a timely manner.
- Assist with bill payments and maintain accurate records of all transactions.
- Provide transportation for clients, as needed.
- Support and collaborate with Fifty Forward Centers and other internal programs.
- Actively identify and maintain strong relationships with community partners.
- Conduct outreach events to promote services and increase engagement.
- Adhere to the National Association of Social Workers Code of Ethics.
- Perform other duties as assigned.
Education:
Bachelor's degree in social work or a related field preferred. Three years of equivalent experience may substitute for formal education.
Preferred Experience:Care management, direct care, legal practice, work related to older adults, governmental benefits, and/or related social services.
SpecialSkills:
Interest in serving older adults and supporting their ability to live independently. Familiarity with trauma-informed care, motivational interviewing, and culturally competent practices. Strong interpersonal and problem-solving skills; ability to work independently and collaboratively. Basic knowledge of Middle Tennessee social service networks. Reliable transportation, good driving record, and vehicle liability insurance required. Bilingual (English/Spanish) fluency is a plus.
Certifications and Licenses:Valid driver’s license required. Willingness to obtain CPR and First Aid certification.
Physical Aspects:Requires travel within Middle Tennessee, primarily Davidson County.
Must be able to visit private residences and institutional settings, which may present environmental challenges (e.g., odors, pets, clutter). Must be able to ascend/descend stairs, bend, push, pull, stand for extended periods, and lift or carry up to 25 pounds.
Equipment Requirements:Ability to utilize a computer, printer, copier, and telephone.
Working Hours:Full-time, 40 hours per week. Standard hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional early mornings, evenings, or weekends as needed.
