About the Role:The Case Manager – Support Services serves as a key member of the I Am Next support services team and provides direct, trauma-informed support to transition-aged youth and young adults with histories of foster care, housing instability, and complex trauma.
This position works collaboratively with participants to identify goals, develop individualized service plans, and support progress toward housing stability, self-sufficiency, emotional wellness, and long-term independence. The Case Manager provides relational support while coordinating access to housing resources, transportation, education, employment, healthcare, behavioral health services, and other community-based supports.
Using strengths-based, motivational, and trauma‑informed approaches, the Case Manager empowers young adults to navigate barriers, build resiliency, and develop the skills necessary for successful adulthood.

- Bachelor's degree?in?
Social Work, Human Services, Psychology, Sociology, Counseling, Education?or related field. - Minimum two years of experience working with transition‑aged youth, foster youth, housing programs, shelters, or community-based services.
- Strong understanding of trauma, attachment, adverse childhood experiences, and the transition from foster care to adulthood.
- Experience with case management, service coordination, coaching, or supportive services.
- Excellent verbal and written communication skills.
- Strong organizational, documentation, and time management skills.
- Ability to remain calm and emotionally regulated in high‑stress situations.
- Proof of Automobile Insurance and Reliable Transportation
- Valid driver’s license
- TBRI Practitioner or Motivational Interviewing trained preferred.
- Experience working with youth aging out of foster care preferred.
- Experience with permanent supportive housing or homelessness services preferred.
- Experience with HMIS, Hub Spot, Salesforce, or other case management systems preferred.
