The Intake & Case Manager – Clinical and Community Programs is the primary point of entry for people seeking support at Black Mental Health Village, responsible for welcoming participants, completing intakes, coordinating referrals, and providing short‑term case management across BMHV’s clinical and community healing spaces. In this full-time exempt role, the Intake & Case Manager also helps pilot BMHV’s technology systems for intake, care coordination, and data tracking so the organization can better understand needs, strengthen its internal referral pipeline, and ensure participants move smoothly into the healing pathways that fit them best.

Position Type: Full-time, exempt

Reports To: Executive Director and/or Clinical Program Lead

Salary Range: $43,000-$53,000 annually, commensurate with experience and qualifications.

Black Mental Health Village (BMHV) is a Black‑led liberatory wellness ecosystem rooted in Nashville. BMHV brings together clinical, communal, cultural, and systems‑level healing to support Black communities and advance health justice.

Position Summary

The Intake & Case Manager will support BMHV’s clinical department and community programming by serving as a first point of contact for participants seeking services, completing intake and assessment processes, coordinating referrals, and providing short‑term case management support. This role will also help pilot BMHV’s technology systems for intake, referral tracking, case coordination, and data capture so the organization can strengthen service delivery and better understand community needs.

The position bridges clinical and community‑based healing work. The person in this role will help participants move across BMHV’s internal healing pathways, including clinical services and other healing departments, based on participant goals and interest.

Core Responsibilities Intake and participant onboarding
  • Serve as a welcoming first point of contact for individuals, families, and community members seeking BMHV services.
  • Conduct intake interviews by phone, virtually, and in person as needed.
  • Complete intake forms, releases, consent documentation, screenings, and service eligibility steps in alignment with organizational policies.
  • Explain BMHV service options clearly and help participants identify the supports that best fit their needs.
Case management and care coordination
  • Provide short‑term case management and care coordination for participants connected to clinical and community programs.
  • Develop and track individualized support plans tied to participant‑identified goals.
  • Coordinate warm handoffs to therapists, support groups, healing circles, youth programs, and other internal BMHV offerings.
  • Connect participants to external community resources when needs fall outside BMHV’s direct services.
  • Maintain follow‑up contact to reduce drop‑off and support continuity of care.
Internal referral pipeline support
  • Help build and maintain an internal referral pipeline across BMHV healing departments.
  • Work with program and clinical staff to ensure participants can move across services smoothly and with informed consent.
  • Flag barriers, delays, and service gaps that affect participant access or engagement.
Technology and data systems pilot
  • Use BMHV’s technology platform(s) to document intakes, referrals, service connections, follow‑up activity, and outcomes.
