LTSS Service Coordinator
This field‑based role serves Williamson County, TN and requires travel to client sites. Occasional office attendance for meetings or training is required.
ResponsibilitiesThe LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs:
- Conduct face‑to‑face program assessments using pre‑defined tools.
- Apply motivational interviewing techniques for evaluations, coordination, and management of individual waivers such as LTSS/IDD, BH, or PH needs.
- Identify and coordinate clinical health care needs, serving as the single point of contact with clinical and interdisciplinary teams.
- Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities to ensure efficient and cost‑effective utilization of long‑term services and supports.
- Document short‑ and long‑term service and support goals in collaboration with the member’s chosen care team, which may include caregivers, family, natural supports, service providers, and physicians.
- Identify members that would benefit from an alternative level of service or other waiver programs.
- Serve as mentor, subject matter expert, or preceptor for new staff and assist in formal training of associates.
- Submit utilization/authorization requests to utilization management with supporting documentation aligned to the care plan.
- Report critical incidents to appropriate internal and external parties, including state and county agencies such as Adult Protective Services and law enforcement.
- Assist and participate in appeals, fair hearings, member grievances, and state audits.
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience as required by state law and contract.
- BA/BS degree in a health care related field.
- Willingness and ability to travel to work sites and other locations as necessary.
