Pay: $45,000 - $52,000 per year, based on experience
Youth Opportunity is a nationally recognized leader in residential and behavioral health services for youth. Harpeth Primary Assessment Center in Nashville, Tennessee, is a secure, 24-bed assessment center serving girls ages 10 to 17.
The center supports the Tennessee Department of Children’s Services by providing temporary, trauma-informed care, observation, and comprehensive evaluation services. These assessments help identify each youth’s strengths, treatment needs, and most appropriate next placement.
Youth receive structured support in a safe and compassionate environment while participating in mental health and substance use assessments, educational services, individualized programming, and transition planning. The program’s goal is to develop informed recommendations that promote healing, stability, and long-term success.
We are hiring a Case Manager to coordinate assessment services, documentation, communication, and transition planning at Harpeth Primary Assessment Center. Working under the clinical supervision of a licensed mental health professional, you will serve as an important liaison among youth, families, the Tennessee Department of Children’s Services, the treatment team, and other community partners.
This position is ideal for a compassionate and organized human services professional who wants to help youth successfully navigate a critical period of assessment and transition.

- Provide case management services and maintain ongoing communication with youth, families, agency representatives, and members of the treatment team
- Coordinate or arrange required mental health and substance use screenings and assessments
- Complete performance summaries, treatment plan reviews, transition plans, aftercare plans, and discharge summaries
Maintain accurate, timely, and objective case records according to organizational and contractual requirements - Serve as a liaison among the program, the Tennessee Department of Children’s Services, outside agencies, and each youth’s family
- Prepare youth and families for program visits, family conferences, transition planning, and aftercare responsibilities
- Connect youth and family members with appropriate community resources
- Assist with post-release and follow-up data collection as required by policy, Medicaid guidelines, and contractual obligations
- Participate in treatment team meetings and communicate each youth’s progress, needs, and placement recommendations
- Collaborate with leadership on performance-improvement activities
- Conduct periodic case-record reviews to support compliance with Youth Opportunity policies and agency requirements
- Maintain a high standard of role-model behavior for youth
- Participate in required training and professional development activities
- Report any acts, incidents, or conditions reflecting the possibility of inappropriate youth-to-youth or staff-to-youth relationships
