Job Coach, Social Work, Human Services​/ Social Work

Sevita is seeking a compassionate Job Coach to support individuals in achieving greater independence and employment success in home and community settings. In this role, you will provide person-centered coaching, teach daily living and job-readiness skills, and help individuals build confidence and meaningful routines. You'll collaborate with clinical teams, families, and community partners, track progress, and advocate for clients' goals. Responsibilities

• Provide person-centered coaching to individuals receiving home and community-based services to support independence and life goals
• Develop, implement, and update individualized service and coaching plans based on assessments and client input
• Teach daily living, social, vocational, and coping skills through structured sessions and real-life practice
• Collaborate with clinicians, case managers, families, and community partners to coordinate care and resources
• Monitor, document, and report client progress, behaviors, and incidents in accordance with agency and regulatory standards
• Advocate for client rights, inclusion, and access to community supports and healthcare services
• Maintain professional boundaries, confidentiality, and compliance with healthcare and social service regulations
• Participate in ongoing training, supervision, and team meetings to improve service quality and outcomes
