As a Blakeford Social Worker, you will counsel residents and their families on emotional, environmental or physical observations and interventions. Conduct regular face-to-face visits with assigned residents, facilitate active participation in care planning, serve as resident liaison for social, health, and community services that meet individual needs throughout discharge planning activities, and compile performance metrics and present summary data. If you have a bachelor's degree in social work, at least one year similar experience, the ability to communicate effectively across multiple generations in a manner that supports independence and dignity and conveys compassion, patience, and inclusion, and a heart for serving seniors and their families, apply today and join a team built on community, compassion, consistency, empowerment, innovation, integrity, and respect.
