Are you able to support an individual to be successful at home, work, and community? If so, you may find fulfillment in the role of a Family Model Provider.

• HS Diploma/GED preferred
• 18 years old or older
• Documentation of home ownership: current mortgage statement or renter’s lease
• Proof of homeowner’s or renter’s insurance or personal property insurance
• Valid Driver’s License; reliable transportation with valid vehicle insurance
• Ability to pass criminal background and Registry checks as well as a drug screen
• TB screenings for all members of household
• Willing to be a self‑employed worker

• Seek to understand the individual in the context of their personal history, social and family networks, and hopes and dreams for the future.
• Support the individual with eating nutritious meals and snacks consistent with dietary needs, resting by providing sleeping accommodations, daily self‑care activities, leisure/recreation, education and working.
• Provide supports according to the person’s abilities and interests while ensuring privacy and dignity.
• Prepare, teach and assist the person in food preparation and cleaning of the home.
• Honor the choices, preferences and culture of the person you support.
• Assist in providing opportunities for social, emotional and spiritual needs to be met.
• Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.
• Ensure that the home environment is safe and in good condition.
• Implement services and supports to address goals identified through the Person‑Centered Plan, including medical, psychiatric, recreational, spiritual and emotional needs.
• Assist the person supported by attending team meetings and facilitating communication related to individual needs and progress toward goals.
• Follow state guidelines specific to Family Model Services.

• Provide multidisciplinary team support for crisis assistance and training as needed; training includes RHA’s proprietary Pro Act training focused on resilience, emotional stability and problem solving.
• Supervision will be provided to assist you in adjusting to your new role; frequency and amount of supervision will be determined collaboratively with your RHA supervisor.

• Complete criminal background check and name checks in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities).
• Drug testing.
• Education verification and other credentialing based on position requirements.
• Proof of employment history or references (if required).
• Positions that require driving: proof of driver’s license, driver's insurance, and vehicle if required for providing transportation.
