We are seeking a compassionate, knowledgeable, and dedicated Social Services Director to join our team and support the social, emotional, and psychosocial needs of our residents.

- Oversee and coordinate the social services program to ensure residents receive appropriate support and resources.
- Advocate for residents and assist with addressing their individual needs, concerns, and preferences.
- Complete and maintain accurate social services documentation and assessments in accordance with applicable regulations and facility policies.
- Assist residents and families with adjustment to the facility and provide support throughout the resident’s stay.
- Coordinate care and communicate effectively with residents, families, staff, and outside service providers.
- Participate in care planning and interdisciplinary team meetings.
- Assist with discharge planning and coordinate appropriate community resources and services.
- Maintain compliance with federal, state, and local regulations related to social services and long-term care.
- Bachelor’s degree in Social Work, Human Services, or a related field preferred.
- Previous experience in long-term care, skilled nursing, healthcare, or social services preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to advocate effectively for residents while maintaining professionalism and confidentiality.
- Knowledge of long-term care regulations and resident rights is preferred.
- Compassionate, dependable, and able to work effectively with residents, families, and interdisciplinary teams.
