Impact Health is looking for a Psych Nurse Practitioner to join our team in Naugatuck, CT. We specialize in Medical, Psychiatric Services, Wound Care, and Palliative Care across the Northeast and prioritize patient care.

Are you an experienced NP seeking a flexible, well‑supported role? This position is specifically for weekend coverage in our friendly skilled nursing facilities and provides autonomy with support to deliver independent care, including physician‑level consult access.

• Deliver person‑centered care using evidence‑based practices
• Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
• Provide assessment, diagnostic, and treatment services
• Provide psychoeducation and medication education to patients and/or families
• Collaborate with interdisciplinary facility teams
• Complete timely documentation (within 24 hours)
• Ensure compliance with state regulations in SNF settings

• Licensed Psychiatric Nurse Practitioner in the state of practice
• Experience in skilled nursing facilities, hospitals, residential care, geriatric services, and outpatient psychiatric settings preferred
• Minimum of 2 years of clinical experience providing mental health care to adult populations
• Proficient with EHRs and clinical technology
• Strong communicator with a collaborative mindset
