Falls City Nursing & Rehabilitation Center is seeking a qualified Occupational Therapist to provide skilled, evidence‑based rehabilitation services to residents with cognitive and/or ADL deficits. This PRN position offers the opportunity to join a collaborative team focused on clinical excellence and resident‑centered care.

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop individualized plans of care in accordance with regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to guide clinical interventions.
• Administer rehabilitative treatments, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
• Assess patient progress and adjust treatment plans accordingly in the clinical record.
• Evaluate treatment outcomes and modify services as needed, making referrals when appropriate.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Communicate regularly with supervisors and other health team members about patient progress, barriers, and treatment plans.
• Prepare comprehensive discharge summaries in compliance with regulatory and clinical requirements.

• Licensed Occupational Therapist in the state of Nebraska
• Experience level is not restricted; openings are available for OTs at all experience levels.
• Demonstrated commitment to patient safety, infection control, and professional collaboration.
