Falls City Nursing & Rehabilitation Center – Speech/Language Pathologist – PRN

Falls City, NE

• Screen and evaluate patients with communication, cognitive, or swallowing disorders; develop appropriate treatment plans in accordance with regulatory and clinical practice standards.
• Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with communication, cognitive, and/or swallowing disorders per regulatory and clinical practice requirements.
• Assess treatment outcomes for effectiveness; modify services in relation to patient evaluation and refer when appropriate.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Prepare comprehensive discharge summaries of services per regulatory and clinical requirements.
• Document all regulatory and clinically required information in the electronic documentation system; maintain patient records per company policy.

• Licensed Speech–Language Pathologist.
• Open to SLPs at all experience levels; new graduates welcome.
