Objective of the role:

The nurse coordinator in eligibility participates in the implementation and coordination of the home care project in collaboration with medical and paramedical health professionals.

Missions:
Evaluation of the admission request:

• Processing of PEC request files according to urgency
• Elaboration of the patient's personalized project: individualized care and life project
• Search for appropriate caregivers for the care plan
• Creation of the nursing care plan

Development and follow-up of the activity:

• Setting up staff with our referring institutions
• Development of new partnerships with hospitals, ESMS, and local networks
• Individual follow-up of the activity

Participation in the continuity of care Management of emergencies

Schedule:

Monday to Friday in a 7:00 AM position with on-call system.
