The Long Term Care (LTC) Activities Assistant strives to improve the quality of life for patients and residents by providing appropriate, meaningful activities on a one‑on‑one and/or group basis. The role includes administering the monthly activity calendar, eliciting resident input for planning, and ensuring activities such as music, craft, and spiritual sessions are conducted per state requirements. Additional duties involve supporting patients during transitions, assisting with nursing procedures, and completing MDS/EPIC documentation.

The assistant demonstrates creativity, follows instructions under supervision, and maintains excellent communication skills.

• High school graduate or equivalent preferred.
• Experience in social services or creative activities preferred.
• Experience in geriatrics preferred.
• Knowledge of working with a wide variety of resident demographics.
• Preferred: current Certified Nurses Assistant (CNA) certification or enrollment in a healthcare‑related program.
• Basic Life Support (BLS) required based on location; all new employees will receive CPR training and must be certified within six months of hire.
• Depending on location, the role may classify as a Sanford Category I or II Driver requiring a valid driver’s license and a motor vehicle report, and must be medically qualified to perform driving functions per Sanford Driving Policy.
