Ponderosa Villa is actively looking to hire a full-time MDS Coordinator in a skilled nursing facility (SNF). Job Description
  • Responsible to assign ICD-10-CM diagnosis codes to the residents' medical conditions
  • Accountable for case management of Medicare Part A and insurance case management
  • Expertly assess nursing home residents' needs, and coordinate personalized, resident-driven care based on those assessments
  • Oversee the patient assessment process, setting assessment schedules and assuring that assessments are done in an accurate and timely manner
  • Make recommendations and transmit assessments to the Dept. of Health. Review the results to make policy and procedural recommendations to the management staff
  • Coordinate patient care plans in accordance with regulatory requirements. Ensure that appropriate facility resources are made available to patients and that patient care is delivered effectively
  • Create the schedule for all Medicare and Medicaid including:
  • Start Medicare coverage for newly qualified patients and/or send out denial letters
  • Keep updated on regulatory changes in Medicare coverage and help determine documents needed for Medicaid reimbursement
Requirements
  • Minimum associate degree. Bachelor's degree preferred
  • Must be a Registered Nurse in good standing
  • Extensive knowledge in MDS
  • One year of MDS experience in a skilled nursing/long-term care facility
  • Extraordinary attention-to-detail, capable of directing other people and managing schedules
  • Excellent communication skills, expert at working cooperatively as a member of a team
  • Proficiency in MDS 3.0
  • Demonstrable working knowledge of relevant state and federal regulations
  • Resident Assessment Coordinator certification
