Opportunity to work with an in‑house therapy team in a skilled nursing facility that offers long‑term care, post‑acute rehabilitation, and geriatric outpatient therapy services.

• Utilize evidence‑based practice and provide rehabilitative and medically necessary treatment interventions under the direction of a Physical Therapist.
• Administer rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices, under supervision.
• Assess patient progress and communicate with the supervising Physical Therapist to adjust treatments accordingly.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Assist in preparing discharge summaries and other required documentation.
• Document all required regulatory and clinical information electronically and maintain patient records per company policy.

• Licensed Physical Therapist Assistant.
• Prior experience in a skilled nursing setting preferred; new graduates welcome.
