The Ponca Tribe of Nebraska is seeking a community-based health outreach worker to implement outreach health care services and health promotion within the Ponca service area.
You will conduct trainings, provide in-home visits, coordinate care, arrange appointments and transportation, and work with the Population/Public Health Department to reduce barriers and improve access to care for Native Americans.
