Job Title: Physical Therapist (PT)
Job SummaryWe are seeking a compassionate and skilled Physical Therapist (PT) to provide high-quality, patient-centered rehabilitation services in a healthcare setting. The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, implementing therapeutic interventions, and monitoring patient progress to restore mobility, reduce pain, improve function, and enhance overall quality of life. The PT collaborates with physicians, nurses, and interdisciplinary healthcare teams to ensure safe, effective, and evidence-based patient care.
Requirements- Active Physical Therapist (PT) license in the state of practice.
- Current Basic Life Support (BLS/CPR) certification preferred or required based on facility policy.
- Doctor of Physical Therapy (DPT) degree or equivalent educational requirements as required by state regulations.
- Previous experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty-specific settings preferred.
- Strong clinical assessment, communication, documentation, and patient education skills.
- Ability to work independently and collaboratively within an interdisciplinary healthcare team.
- Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
- Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
- Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
- Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
- Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
- Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
- Ensure compliance with patient safety, infection prevention, and quality assurance standards.
- Treat patients across assigned care settings and age groups as required by the facility.
- Perform other job-related duties as assigned.
