Job Description Position Summary

This position contributes to the fulfillment of Thayer County Health Services vision and mission by providing prescribed medications, drugs, and other pharmaceuticals according to professional standards and practices.

Reports to Retail Pharmacy Manager and Director of Pharmacy.

Job Qualifications

Pharmacy degree from an American Council for pharmacy Education (ACPE) accredited school of pharmacy and licensed through the successful passage of a licensure exam in accordance with National Association of Boards of Pharmacy. PharmD preferred. Three years of retail experience as a Pharmacist preferred. Must have good communication skills; both written and verbal. Requires current state license to practice pharmacy.

Principal Responsibilities

Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes by advising physicians on drug therapy, toxicity, side effects, and assisting in prescribing appropriate doses. Ensures adequate quantities of drugs and supplies are ordered and maintained.

Prepares and dispenses all medications, oversees non-pharmacist personnel in the medication use process, and ensures all prescription and medication orders are accurate, complete, and appropriate for the patient in compliance with state and federal legal requirements.

Maintains relationships with medical and nursing staff, providing timely information on pharmaceuticals, therapeutic options, drug usage and compatibility, state and federal regulations, and Nebraska Department of Health standards.

Conducts and evaluates medication use reviews, assesses patient compliance and outcomes, and suggests modifications to achieve desired outcomes; instructs patients on proper use of prescribed drugs and performs patient care rounds with physicians as needed.

Individualizes medication regimens using sound principles, accounting for pharmacodynamics and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination, and continually improves delivery of pharmaceutical care within the department.

Ensures that medicinal products are stored appropriately and securely, maintaining inventory in accordance with state and federal requirements.

Responds to medication-related queries from other hospital departments and the general public.

Maintains strict confidentiality and complies with HIPAA recommendations and regulations.

Keeps up to date with, and contributes to, changing drug knowledge and healthcare developments in collaboration with medical staff and colleagues.

Writes guidelines for drug use within the hospital and long-term care facilities, prepares drug and therapeutic bulletins, and implements necessary regulations and related policies.

Provides information related to budgets and drug expenditures.

Processes all cash and credit transactions as required, providing patients with proper receipts and information.

Attends required meetings and participates in facility and community committees as requested.

Mentors healthcare professionals in training, including pharmacy, medical, and nursing students, in both practical and didactic experiences.

Provides monthly consulting to contracted LTC facilities.

Performs other duties as needed by the facility.

Maintains and promotes good health practices, conforms to TCHS procedures and policies, and performs all duties as described within the scope of licensure. Does not discuss patient matters outside the clinical setting, ensuring HIPAA compliance. Understands and complies with Thayer County Health Services Compliance Program, the Code of Conduct, the Compliance Manual, and all supporting policies. Participates in education and training programs regarding compliance as required.
