Key Rehab is looking for a full‑time Program Coordinator-OTA or PTA to join our team in providing excellent care in the Humboldt area in a SNF setting.

  Completion of an accredited PTA or OTA program.

  Current PTA or OTA license in the state(s) assigned.

  CPR, as required by the community.

Preferred:
Prior experience in a long‑term care setting and service coordination.

In addition to providing therapy under their disciplinary scope of practice (OTA or PTA), the Program Coordinator serves as the liaison between Key Rehab and community staff. This position is instrumental in minimizing administrative burden and providing support to allow clinicians to deliver therapy services. The coordinator also manages staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor.
