About the Role:
As a Per Diem Speech‑Language Pathologist, you will be responsible for providing high-quality speech‑language pathology services to our residents in a skilled nursing facility setting. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans, conduct comprehensive assessments, and provide evidence‑based therapy interventions. Your expertise will be essential in helping our patients overcome communication and swallowing disorders, promoting independence, and enhancing their overall quality of life.
Responsibilities:
- Conduct comprehensive assessments of residents' communication and swallowing abilities
- Develop and implement individualized treatment plans in collaboration with the interdisciplinary team
- Provide evidence‑based speech‑language therapy interventions, including individual and group therapy sessions
- Monitor and document resident progress, making adjustments to treatment plans as needed
- Communicate effectively with residents, families, and healthcare staff to ensure excellent care and support
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
Requirements:
- Master's degree in Speech‑Language Pathology (CCC‑SLP) from an accredited program
- Current state licensure and certification as a Speech‑Language Pathologist (CCC‑SLP)
- Experience working with older adults and individuals with communication and swallowing disorders
- Ability to work independently and as part of a team
- Excellent communication and documentation skills
