Responsibilities- Provide personalized care and assistance with daily living activities to clients in their homes, including bathing, dressing, grooming, and light housekeeping
- Assist clients with meal preparation, medication management, and mobility exercises
- Develop and maintain strong relationships with clients, their families, and healthcare professionals to ensure comprehensive care
- Document care plans, progress notes, and other relevant information in accordance with company policies
- Collaborate with the care team to develop and implement effective care plans that meet each client's unique needs
