About the Role:As a Home Health Aide with Modani Care, you will provide direct patient care, assist with daily living activities, and promote independence and well-being for our clients. Your compassionate and dedicated approach will help us deliver exceptional care that exceeds our patients' expectations.
Responsibilities:- Assist patients with daily living activities, such as bathing, dressing, and grooming
- Provide medication reminders and assistance with medication management
- Help with meal preparation and light housekeeping
- Assist with transferring patients to and from wheelchairs or beds
- Monitor patients' conditions and report changes to our healthcare team
- Maintain accurate and detailed records of patient care and activities
