Henderson Health Care Services, Inc., a progressive, health care facility located in Henderson, NE, is seeking an on-site, full time HIM Clerk. The HIM Clerk is responsible for organizing, reviewing, and abstracting medical record information to ensure the accuracy, completeness, and confidentiality of patient health records. This position plays a key role in supporting documentation integrity and regulatory compliance within the HIM Department.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assemble and organize patient medical records, placing documentation in the correct order and sequence.
  • Analyze records to identify missing documentation; flag incomplete entries or those requiring signature or dictation.
  • Prepare patient charts by gathering and verifying documentation from clinical encounters.
  • Notify providers of incomplete or missing dictation, ensuring timely completion per medical staff bylaws.
  • Monitor provider documentation and report completion status to maintain compliance with record completion standards.
  • Track and report documentation deficiencies to help ensure timely completion.
  • Maintain effective and professional communication with providers to support workflow and documentation processes.
  • File, scan, and index health records into the electronic medical record (EMR) system in accordance with departmental policies.
  • Respond to internal and external medical record requests in full compliance with HIPAA regulations and organizational policies.
  • Handle the release and receipt of health information securely and accurately.
  • Collect, organize, verify, correct, and distribute health record data to maintain complete and accurate medical charts.
  • Ensure the security and confidentiality of all protected health information (PHI) within the HIM department.
  • Report discrepancies or issues in patient records to the HIM Director for timely resolution.
  • Review all work for accuracy, compliance, and conformance with policies and procedures.
  • Maintain awareness of and compliance with Federal and State medical record regulations, including Medicare documentation requirements.
  • Remain flexible and willing to adjust schedule or tasks based on department needs.
  • Follow all established HIM department policies and procedures.
EDUCATION and/or EXPERIENCE

High school diploma or general education degree required.
