Job Description Purpose of Your Position

The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises day‑to‑day care provided by Certified Nursing Assistants (CNAs) when an RN is not available. The LPN ensures that all nursing activities comply with federal, state, and local regulations—maintaining the highest standard of quality care at all times under the direction of the Director of Nursing Services or Nurse Supervisor.

Duties and Responsibilities
  • Direct and supervise nursing assistants in accordance with established policies, rules, and regulations.
  • Participate in shift planning, services, and daily unit activities.
  • Ensure compliance with facility policies and procedures among assigned staff.
  • Complete required reports, forms, evaluations, and charting accurately and on time.
  • Participate in government agency surveys and inspections as needed.
  • Admit, transfer, and discharge residents as required.
  • Document incidents, accidents, and changes in resident condition promptly.
Charting and Documentation
  • Transcribe physician orders and update charts, Kardex, and care plans.
  • Maintain accurate, descriptive nurses’ notes reflecting care provided and resident response.
  • Complete and submit required documentation for accidents, incidents, and evaluations.
  • Follow facility policies for all charting and recordkeeping requirements.
  • Accurately administer prescribed medications and treatments.
  • Ensure adequate supplies and equipment are available for resident care.
  • Maintain accurate narcotic records for each shift.
  • Report medication errors and discrepancies to the Director of Nursing.
Leadership and Quality
  • Provide leadership and guidance to nursing assistants and support staff.
  • Conduct daily rounds to ensure quality and consistency of resident care.
  • Create and maintain an atmosphere of warmth, respect, and teamwork.
  • Give and receive shift reports accurately and thoroughly.
  • Make rounds with physicians as needed and assist with resident assessments.
  • Notify physicians and family members of changes in resident condition or incidents.
  • Administer treatments and nursing services including wound care, catheterization, tube feeding, and specimen collection.
  • Monitor seriously ill residents and provide attentive care as directed.
  • Assist with end‑of‑life and post‑mortem procedures in accordance with facility policy.
Safety and Sanitation
  • Ensure assigned staff follow safety protocols and infection control procedures.
  • Maintain clean and organized nursing stations and treatment areas.
  • Report unsafe conditions or equipment immediately to the Nurse Supervisor or Director of Nursing.
Care Plan and Assessment
  • Review care plans daily to confirm that resident care aligns with individualized goals.
  • Ensure CNAs understand and follow each resident’s care plan.
  • Update nursing notes and care plans as needed to reflect resident progress and condition changes.
