Location: Du Bois
Penn Highlands Healthcare is seeking a Licensed Practical Nurse to support call center and patient care coordination in transitional and chronic disease management. You will assist in developing and evaluating patient care plans and coordinate with diverse health care providers.
The role includes serving as a liaison among physicians, mid-level providers and external health care teams. A PA LPN license and CPR within first year are required, with preferred office experience.
