LPN - Licensed Practical Nurse - Night Shift

Role Overview: The Licensed Practical Nurse (LPN) provides professional nursing care to residents in a long-term care setting, under supervision of a registered nurse, advanced practice provider, or physician.

• Assess residents and administer medications or treatments as prescribed.
• Perform testing to evaluate resident status and respond to emergent situations.
• Ensure residents' health, comfort, and safety while documenting care activities and outcomes.
• Support interdisciplinary team collaboration and communicate effectively with residents, families, and healthcare providers.
• Counsel residents and families on prevention and treatment plans and develop self-care programs.
• Maintain confidentiality, establish effective working relationships, and perform additional duties as assigned.

• Graduate from an accredited practical nursing program; associate degree preferred.
• Maintain an unencumbered LPN license and meet department-specific competency requirements.
• Possess IV therapy skills if required, with relevant coursework or experience.
• Minimum 400 hours of clinical experience or equivalent work experience as an LPN.
• Strong communication, problem-solving, and teamwork capabilities.
• Commitment to ongoing education and certification maintenance.
