Position SummaryThe Physical Therapist provides skilled, resident-centered care in a Skilled Nursing Facility, evaluating and treating Residents to improve mobility, function, and independence. This role collaborates closely with Residents, families, and the interdisciplinary care team to design and deliver effective therapy plans that support meaningful recovery outcomes.

- Treats Residents according to treatment plan
- Records clinical documentation according to accepted regulatory and professional guidelines
- Evaluates Residents based upon the physician referral and orders
- Develops effective treatment plans with functional, goal based outcomes and ensures approval for services from referring physician
- Supervises Therapist Assistants and therapy students in direct Residents care to carry out plan of care and Residents related activities per state practice act and reimbursement regulations
- Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations, provides payer updates as requested
Records daily treatment charges per approved billing guidelines - Instructs families and nursing staff regarding restorative, home programs, Resident specific plan of care
- Participates in discharge planning, care planning, MCR meeting and QA
- Communicates with supervisor and other clinical Team Members regarding Resident progress, problems and plans
- Participates in interdisciplinary team meetings, Resident care conferences, utilization review meetings, and other meetings as required
- Coordinates with members of other departments, attending physicians, committee members, and other staff within the Rehab department
- Screens Residents based upon identified needs
- Assists with cleaning maintenance of treatment area and department
- Secures necessary durable medical equipment and adaptive equipment for Residents
- Reports any problems with department equipment to appropriate personnel
- Provides coverage cross campus as needed
- Recognizes and reports all allegations of Resident abuse, neglect, exploitation, social media exposure, and/or misappropriation of Resident property immediately
- Meets department uniform, appearance and grooming requirements
- Complies with all OSHA safety regulations, local applicable laws regarding health, safety or environment

- Ability to maintain confidentiality and strong customer support and communication skills
- Ability to work independently with limited supervision
- Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public
- Work professionally with people from a wide variety of cultural and socio-economic backgrounds
- Complies with protection of health information (HIPAA) safeguards
- When working in Resident areas, must be able to communicate effectively in English
- Attends and participates in in-service educational classes, memory care training, Healthcare Academy and staff meetings as directed
A license in Physical Therapy for the state of Massachusetts is required
Current CPR and AED certification required
Proficient with computer software programs and EMR
