Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapy licensure in the state of Massachusetts.
• Current CPR certification.
• Current driver's license, vehicle insurance, and access to a dependable vehicle for patient visits.
• Experience in home health settings, with emphasis on post-acute hospitalization and assisted living populations preferred.
• Ability to work autonomously and onboard quickly preferred.
• Minimum education:

  evaluate functional status and systems for CMS documentation, develop PT plan of care, provide evidence-based treatment to relieve pain and restore/maintain function, document progress notes per policy, and communicate changes to nursing and physicians.
