Community-Based Case Manager

Position Overview The Community Based Case Manager is responsible for:

Developing and implementing Individual Service Plans (ISP).Supporting community inclusion and independent living skills.

Providing care coordination, direct care, and supervision as needed—including personal care and daily living activities.

PRIMARY DUTIES:

Coordinate care and provide support in:

Education and social skills

Meal planning and grocery shopping

Personal care and hygiene

Clothing care and travel training

Money management and housing

Community participation and religious practices (if desired)
Transport individuals to medical appointments and:

Follow up on medical care plans

Educate individuals, families, and staff on self-directed care Support healthy, culturally appropriate meal planning and purchasing

Provide transportation and skill-building in grocery shopping REQUIRED SKILLS AND RESPONSIBILITIES:

Gathers and shares relevant data as indicated by the family or CRC Support Navigator.

Bachelor’s degree (or currently enrolled) in Human Services or a related field

Equivalent experience may be considered in some cases

Preferred:
Experience working with individuals with developmental disabilities

Familiarity with behavior modification techniques

Strong oral and written communication skills

Valid Massachusetts Driver’s License and reliable personal transportation

Proficient in Microsoft Office and general computer use LANGUAGE

SKILLS:

Fluent in English with the ability to read and interpret documents.

Ability to communicate effectively with clients, colleagues, families, and community members.

Strong written and verbal communication skills for reports, correspondence, and presentations.

PHYSICAL DEMANDS:

Employee must have significant endurance to sustain the physical demands of the role.

Regular activities include sitting, standing, walking, stooping, kneeling, or crouching.

Frequent use of hands/fingers to handle objects and tools.

Ability to generate force to maneuver up to 100 pounds (assistance provided when possible).Must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead.

REASONING ABILITY:

Ability to follow detailed written and oral instructions.
