Duration: 13 weeks (contract)
Schedule: Full-time, 40 hours/week (Mon-Fri, 8:00 AM - 4:30 PM) Optional weekend rotation available

Position Overview
We are seeking experienced Home Health Registered Nurses (RNs) to provide high-quality, patient-centered care in a home health setting. This role involves managing patient care plans, conducting comprehensive assessments, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.
Coverage Areas (4 Openings)
Candidates will cover one of the following territories:
- Option 1: Merrimac, Haverhill, Methuen, Lawrence, North Andover, Andover, North Reading, Wilmington, Burlington, Bedford, Concord, Acton, Carlisle, Billerica, Chelmsford, Westford, Tyngsborough, Dunstable, Dracut, Lowell, Tewksbury
- Option 2: Mattapan, Jamaica Plain, Roslindale, West Roxbury, Hyde Park, Brookline
- Option 3: Arlington, Belmont, Lexington, Lincoln, Sudbury, Waltham, Watertown, Wayland, Weston, Winchester
- Option 4: Quincy
- Develop, implement, and revise individualized patient care plans in collaboration with physicians and interdisciplinary teams
- Conduct comprehensive patient assessments, including OASIS documentation and medication reconciliation
- Deliver skilled nursing care in accordance with the plan of care and infection control standards
- Supervise and provide guidance to Home Health Aides (HHAs) and LPNs/LVNs
- Perform required supervisory and follow-up visits
- Accurately and timely document all patient interactions per company and payer guidelines
- Coordinate care effectively with all members of the healthcare team
- Active Massachusetts RN License
- Home Health experience required
- OASIS experience required
- Current BLS certification (by start date)
- Valid driver's license
, reliable transportation, and proof of insurance - Ability to travel within assigned territory
- Pay rate: $57/hr
- Mileage reimbursement provided (rate TBD)
