Physical Therapist - Home Health
This is a 13-week contract position.
Night Shift: 5x8, 40 hours per week, with weekend requirement every other weekend.
We are seeking a dedicated Physical Therapist to provide professional rehabilitation therapy to patients in their home. You will be responsible for the evaluation, development, and implementation of individual goal-oriented therapy programs in accordance with organizational policies and procedures.
Responsibilities:
- Complete patient evaluations within the specified time frame, ensuring a holistic assessment of the client's condition.
- Develop individualized treatment programs based on evaluation findings, focusing on measurable short- and long-term goals.
- Prepare appropriate documentation, such as progress notes, reassessments, and discharge summaries, in a timely and precise manner.
- Assure service delivery follows the frequency and duration outlined in the physician's plan of care for the client.
- Act as a liaison among physician, the Multi-Care Physician (MCP), and other healthcare team members.
- Supervise the Physical Therapy Assistant (PTA), ensuring their interventions are performed competently.
- Educate patients and their caregivers on written home programs and provide comprehensive education tailored to their needs.
- Participate actively in discharge planning throughout the patient's course of care.
- Document all case communications appropriately throughout the patient's course of care.
Required Experience / Certifications / Licensure:
- Minimum of 1 year of Physical Therapy experience.
- Certified Home Care experience (Part
A) with OASIS knowledge and current usage is required. - Registered Physical Therapist with BLS certification from the American Heart Association.
- Proficiency in computer documentation and multi-tasking is essential.
