Location: Lockwood

Provide holistic and comprehensive support and case management services to members of the community. This process includes intake assessment, goal setting, plan development, progress monitoring, advocacy, referrals, financial management, grant knowledge, reporting, and overall ability to work with people.

This position is an intermediate-level classification. As such, the position provides administrative support functions at the direction of the supervisor. As experience is gained, the incumbent works more independently within established guidelines. This position level is distinguished by a greater level of proficiency and depth of knowledge that can be applied following clear guidelines or with a higher level of independence.

At this level, guidelines are provided; however, a greater level of judgment is used to apply skills effectively.

  • Any combination of training, education and experience that would provide the required knowledge and abilities. A typical way to gain the required knowledge and ability is:
  • Associate Degree or higher in social science, psychology, or closely related field is preferred;
  • Two years of full-time experience in secretarial or general office support work which has included dealing with the public and populations from different socioeconomic, ethnic and cultural backgrounds; or,
  • Combination of experience and education.
    • Must obtain and maintain a Community Health Worker Certification within six (6) months of hire.
    • Possess and maintain a valid Nevada Drivers License appropriate for vehicles and other equipment used.
    • The ability to pass a criminal background investigation.
    • Military experience applicable to the duties of this position may be preferred.

    The duties listed below are examples of the work typically performed by an employee in this position. Performance of these functions is the reason the job exists. An employee may not be assigned all duties listed and may be assigned duties which are not listed below.

    • Interviews clients to obtain information, determines pre-eligibility for various social service programs and assesses the appropriateness of referral to other community assistance, medical or social service agencies.
    • Assesses the need for specific types of assistance, such as transportation, medical care, housing, appliances for the disabled, senior companion or burial services.
    • Provides outreach to the community or client base to which assigned, meeting and interviewing them in various locations, including, but not limited to, detention centers, medical facilities, clients' homes, on the street, etc.; makes clients aware of services and community resources available to them.
    • Obtains and verifies income, asset insurance and related information; contacts employers, banks, social security, veterans' administration and other public and private sources as necessary.
    • Assists clients in completing applications; provides factual information regarding various program regulations and procedures; may act as an advocate for the client with other organizations.
    • Creates and maintains case files; maintains accurate documentation regarding all information received and actions taken.
    • Maintains accurate logs and complies records of work performed.
    • Contributes to the efficiency and effectiveness of the department's service to its customers by offering suggestions and directing or participating as an active member of a work team.
    • Represents the County with dignity, integrity, and a spirit of cooperation in all relationships with staff and the public.
    • Input accurate and complete data for all contacts with clients into required database
    • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
    • Continuously work with fellow staff to promote excellence as a program, cohesion of service for clients, and to encourage teamwork
    • Provide all required documentation in a timely manner, which may include client assessment, check requests, follow-up, outcome evaluation, grant reporting, client contact sheets, and evaluations.
    • Participate in regular staff meetings, staff-training programs, and accept the responsibility for aiding the development of positive team relationships as requested.
    • Consistently demonstrates positive attitude and progressive actions through the display of professionalism, courtesy, tact, punctuality, attendance, and discretion in all interactions with coworkers, supervisors, and the public; uses common sense discretion with no supervision; remains safe, socially moral, lawful, affective ,adaptive, and efficient.
