Dahlia Hospice is looking for a Medical Social Worker to join our hospice team in Pahrump, NV. At Dahlia Hospice, we are committed to providing compassionate, patient-centered care that supports the emotional, social, and practical needs of patients and their families. Our team works closely together to ensure every patient receives dignity, comfort, and respect throughout their hospice journey.
WhatYou’ll Do
- Provide psychosocial assessments for hospice patients and families, identifying needs related to caregiver stress, family dynamics, safety concerns, coping, and end-of-life planning.
- Offer compassionate counseling and emotional support to patients, families, and caregivers during difficult conversations, crisis situations, and the hospice care process.
- Collaborate with the interdisciplinary hospice team, including nurses, providers, aides, chaplains, and other care team members, to develop and update individualized care plans.
- Connect patients and families with community resources, support services, and local agencies to help address social, financial, emotional, and practical needs.
- Support bereavement services by assisting families through grief and loss and helping coordinate ongoing support after a loved one’s passing.
- Maintain accurate and timely documentation in accordance with hospice standards, company policies, and state and federal regulations.
- Serve as an advocate for patients and families, helping ensure their voices, goals, and preferences are heard and respected.
- Master’s Degree in Social Work required.
- Experience in hospice, home health, healthcare, or senior care preferred.
- Strong understanding of psychosocial needs related to serious illness, end-of-life care, family support, grief, and bereavement.
- Compassionate, patient-centered approach with the ability to support families through sensitive and emotional situations.
- Strong communication and collaboration skills.
- Ability to work independently in the field while staying connected with the hospice care team.
- Cultural sensitivity and the ability to support patients and families from diverse backgrounds.
- Knowledge of hospice regulations and documentation standards preferred.

Hourly range for posted region: $35 - $45 USD.
