Overview

At Dahlia Hospice, we believe every patient deserves comfort, dignity, and compassionate care during life's most important moments. We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) to join our hospice team and make a meaningful difference in the lives of patients and their families. As a Hospice CNA, you will provide hands-on personal care and emotional support to patients in their homes, assisted living communities, and skilled nursing facilities while working alongside an interdisciplinary team committed to exceptional end-of-life care.

Responsibilities
  • Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance.
  • Assist patients with activities of daily living while promoting comfort, dignity, and independence.
  • Observe and report changes in patient condition to the supervising nurse.
  • Provide emotional support and companionship to patients and families.
  • Assist with light housekeeping related to patient care needs.
  • Maintain accurate documentation of services provided.
  • Collaborate with nurses, social workers, chaplains, and other hospice team members to support the patient’s plan of care.
Qualifications
  • Current CNA certification required.
  • Hospice, home health, caregiving, assisted living, or skilled nursing experience preferred.
  • Compassionate, patient-centered approach to care.
  • Strong communication and interpersonal skills.
  • Ability to work independently while maintaining close communication with the clinical team.
  • Valid driver’s license, reliable transportation, and proof of insurance.
