Home Health Physical Therapy Assistant; PTA

Position: Home Health Physical Therapy Assistant (PTA)
Location: Dayton

Eden Health -  - Responsibilities:
Implement treatment plans ordered by the physician as indicated by the plan of care to all patients on caseload;
Review caseload with supervising physical therapist on a regular basis as established by best practice and/or State guidelines;
Educates patients, their families/caregivers, and other staff in appropriate physical therapy modalities;
Delivers physical therapy modalities appropriate to the patients clinical status and needs; in accordance with the standard of care for patients age;
Completes accurate and thorough visit notes via Electronic Health Record on every patient in a timely fashion
